UoW Staff Cardax ‘Access’ Card Authorisation Form

If you qualify for a Staff ID Card please contact HR directly to obtain a new Staff ID Card first, this will become your Staff ID, door access and photocopier card. When you receive your Staff ID Card ask your Line Manager/Administrator to complete this form below on your behalf, quoting your card # (printed on the BACK of your card) in the comments box. Once programmed we will email you with your pin # and instructions.
If you do not qualify for a Staff ID Card then a plain white access card will be sent by us instead

Please note that

- All Cards cost $15.00 and cannot be programmed for a staff member without authorisation and account code. If they don't qualify for a Staff ID card a plain white access card will be sent instead.
- Please allow 3 working days for the application to be processed.
