Application to First Enrol in a Doctoral Programme

Domestic students who have previously been enrolled at the University of Waikato need to complete this form to apply for an MPhil/PhD/DMA/SJD programme. Application to Enrol for Returning Students/Domestic

Domestic students who have not previously been enrolled at the University of Waikato need to complete this form to apply for an MPhil/PhD/DMA/SJD programme. Application to Enrol for New Students/Domestic

International candidates need to go to the International Services webpage to apply to enrol.

Confirmation of Doctoral Enrolment

All candidates applying for confirmed enrolment in a PhD, DMA or SJD programme need to complete this form. Confirmation of Doctoral Enrolment

Progress Report Form

Progress report templates are generated within the Student Database. Please contact the Postgraduate Studies Office for your individual form.

Application for Change of Conditions of Enrolment

Any candidate wishing to change their enrolment by way of suspension, extension, change of status or change of supervisory panel, need to complete an Application for Change of Conditions of Enrolment Form

Request for Overseas Doctoral Study

Any candidate wishing to travel overseas for reasons relating to their research, or to relocate to an overseas address, need to complete a Request for Overseas Doctoral Study Form

Soft Bound Submission

All candidates wishing to submit an MPhil/PhD/EdD/DMA/SJD thesis for examination need to complete a

Hard Bound Submission

All candidates wishing to submit a completed MPhil/PhD/EdD/DMA/SJD thesis in fulfilment of the award need to complete a Hard Bound Submission Form, as well as a Library Authorisation Form
