Application for Review of Grade

Complete this form once your grades are final and you consider a mistake has been made in calculating or determining the grade for your paper(s).

A separate application should be completed for each paper you wish to have reviewed. A standard fee of $65.00 applies to each application. If your application is successful and your grade changes, the $65.00 fee will be refunded.

Include all items of assessment when applying for a Review of Grade.
