| Application for Confirmed Enrolment | This form is completed at the confirmed enrolment presentation and records a doctoral researcher's progress toward confirmation, including submission of the full research proposal, ethics status, self‑evaluation, and the presentation itself; supervisors and the confirmation panel use it to assess readiness for confirmation and document the final outcome. | Download Application for Confirmed Enrolment form (PDF 447kb) |

| Co-Authorship Form | This form is used when a PhD thesis includes material from co‑authored publications or works, documenting the doctoral researcher's specific contribution. | Application for a Change in Supervisory Panel | This form is used by docotral researchers to request a change to their supervisory panel, including adding, removing, or replacing supervisors. | Doctoral Overseas Travel Form | This form is used by international doctoral researchers to notify the University of any temporary overseas travel undertaken during enrolment.
