Incident Report Form

This form must be used for recording all incidents:

  • incidents with students which need to be managed by library staff and/or security (abusive and or threatening behaviour, theft, vandalism etc)
  • incidents where you are concerned about a person’s behaviour

The form must be filled out even when Security are involved.

All incidents must be recorded on this form asap within 24 hours of the incident.

Note: Health and Safety incidents are reported via the H&S reporting system, do not use this form.
