This paper aims to provide registered health professionals with the knowledge and skills to meet the health needs of people with long-term conditions. The paper incorporates the principles of person-centred care and self-care strategies within the wider context of equity, whānau, hapu, iwi and the social determinants of health for people with long-term conditions and co-morbidities.
